I hereby grant my authorization and free, express consent, without this implying any compensation to me, so that MSD:

a) Include in their databases my personal data, such as my name / s, surname / s, email address, date of birth, nationality, gender, cell phone number, landline and / or fax number, identification (if applicable, document that certify the representation of the owner of the personal data), address and specialty, signature, electronic signature and registration number and / or medical registration in the case of HCP (the “Data”) which will be treated in strict compliance with national legislation in this matter and with full respect for MSD's corporate privacy policies.

b) Contact me by any means of communication existing in the present or that could be implemented in the future, such as through email, telephone calls or through any channel and / or digital platform (including, but not limited to, messaging or other similar channels), to provide me with market access content, invitation to events, information about products, commercial information of products and / or services offered by MSD (for example, web shop) or any of its subsidiaries or related companies.

c) Share my Data with subsidiaries or related companies of MSD, which may imply the transfer of the same to countries that have different protection of personal data than Chile, acknowledging that said transfer will occur based on my consent, of according to MSD standards and applicable regulations.

d) Carry out the tasks and / or actions indicated above directly or through third parties.
